Microsoft Visual C++ 6.0 is unhappy:

H:\commoncpp2\tests\thread1.cpp(5) : error C2871: 'std' : does not exist or is 
not a namespace
Error executing cl.exe.

Removing line 5 fixes the problem and g++ on Linux doesn't care.
