I have already implemented it. I'll release a new version soon. But be 
aware that 1) reading backwards large blocks (trimming) is much slower 
than reading forwards, and 2) ddrescue already can skip by great amounts 
(up to LLONG_MAX / 4) if enough consecutive errors happen.

No. Infinite error rate is the only default --max-error-rate that makes 
sense. BTW, 0 and unlimited are not the same thing.
