The last part of buffer *Colors* (from list-colors-display) has these four
lines:
 

SystemTitleText        SystemWindow     #ffffff
SystemWindowFrame      SystemWindowText     #000000
SystemMenuHilight      SystemMenuHilight    #316ac5
SystemMenuBar        SystemMenuBar     #ece9d8
 

The first two of these do not have the same color names for both columns.
 

In GNU Emacs 23.0.92.1 (i386-mingw-nt5.1.2600)
 of 2009-03-30 on SOFT-MJASON
Windowing system distributor `Microsoft Corp.', version 5.1.2600
configured using `configure --with-gcc (3.4)'

I've just installed a fix - there was code to handle this as a special case already when de-duplicating the color table for display, but it was conditioned incorrectly.
