Sorry, not "all".  `do_sgr_italics` and `do_reverse_video` are exceptions. 
But they're pretty similar in that they need only be statically configured
when the output driver initializes.  At present we expose neither device
control commands nor environment variables to manipulate them.  You get
command-line options and that's it.
