That's a bug, one I didn't ever notice because the conditions are unusual: a
single line of paragraph text at the start of a document followed immediately
by an image.  A trap that's 1 unit below the first line isn't being sprung. 
If you add another line to the top, the image is back where it should be.

I'll fix this in the repo, but a quick fix for now is to open the offending
om.tmac (the one from HEAD), search for
  .MAC PDF_IMAGE END
and insert immediately afterwards
  .ch RR_ADVANCE_FROM_TOP
