More complicated than I thought. Maybe for 1.25.
An open question is whether hyphenation exceptions should be bound only to a
language selection or to an environment+language _combination_.
I think the former likely suffices; a multilingual document is going to be
rare enough; one where different hyphenation exceptions are desired for
distinct environments using the _same_ language seems like a stretch, and
could be managed at the _groff_ language level by defining a macro to switch
`hw` exceptions in and out...
...*if* we had the `rhw` request I've been mulling.
