Lily does indeed use hsize to set linewidth (in paper-init.ly).
This is exactly the problem with the current (lack of) support
for landscape format, namely that the user has to specify 
the linewidth manually even if the paper size and orientation
is known.

It used to be that papersize could be set as a global variable
instead of a paper variable, but since some year or more the 
user has to set it as a paper variable since the paper variable
(which is set to a4 by default) overrides the global variable.
The current handling of papersize is a real cludge, in my 
opinion.
