What did you expect LilyPond to do? LilyPond is a program that
is very good at typesetting music but knows nothing about how to
play MIDI files. Note that LilyPond is one of these programs where
you have to read the tutorial to be able to do anything useful at

Those appear to be warnings, not errors, but since no audio output is produced I assume they're the cause of the problem. There is no file named init.midi on my system.

This is OS X:
Darwin localhost 6.6 Darwin Kernel Version 6.6: Thu May 1 21:48:54 PDT 2003; root:xnu/xnu-344.34.obj~1/RELEASE_PPC Power Macintosh powerpc

Lilypond version is 1.6.7.
