That pdf output is not so interesting; it just displays the extenders as you
would expect them to look like (i.e. different from your results.)
The ideas I presented were mainly attempts to eliminate any locale-specific
differences.
To get a minimal example, can you try to remove notes, one by one, until the
bug can be triggered just by removing or adding a single note? (if the score
occupies more than one system, this can be done on each system, so that
adding/removing one note on any system triggers the bug).
(This can usually be done quite efficiently by binary search: If you have 30
notes, then you can remove the 15 first notes; if this _does_ remove the
problem, then the problem probably happens within the first 15 notes, and you
can undo your change, and try removing the last 15 notes instead. If you find
a case where the problem appears with 15 notes, then you can try again to
remove the first 7 notes, and so on.)
