You now need to put some dynamic mark at the start of every voice,
not just the start of every staff.
Heikki reported several MIDI bugs here:
http://lists.gnu.org/archive/html/bug-lilypond/2011-09/msg00987.html
and he fixed one closely-related bug, as of version 2.15.14
