Ncurses license notices

I was just flipping through the ncurses files and found a bunch without
license notices (and a few with bogus notices).  test/* are missing
notices.  form/fty_* has the following notice:


* THIS CODE IS SPECIFICALLY EXEMPTED FROM THE NCURSES PACKAGE COPYRIGHT.
* You may freely copy it for use as a template for your own field types.
* If you develop a field type that might be of general use, please send
* it back to the ncurses maintainers for inclusion in the next version.

This seems to permit *less* than the simple permissive license that most
of the rest of ncurses under -- specifically, it doesn't allow
distribution.  

Many of the files that aren't .c, .cc or .h files are missing notices
too.
