We could add an extra attribute to the image tag. The syntax of the
image tag could be:
<image> =
^@^H[image src="<image file>"
(text="<txt file contents>")?
(alt="<alt text>")?
(dpi="<dots per inch>")?
^@^H]

We could check with Emacs developers if they could make use of such an
attribute or if some other solution would be preferred.
