I found another problem while I was rereading the
fd_buffer_{input,output} code and installed a fix, but I still don't see
how a 0 status can be returned without setting *wrote to the correct
value.  Therefore, I've also installed some assertions to verify
assumptions that could be violated to get this result.

Of course, one of my assumptions is that the problem is in
fd_buffer_output since that is the only buffer implementation that has
changed recently.  It is hard for me to debug this since I don't have
access to a machine that is failing in this manner.
