Until now you had to select an menu item and right click in order to add
to a menu. This was strange, the more obvious thing is to right click
the menu itself in order to add something to it.
I've finally fixed this peculiarity. 
There are still the tooltips of menus to extract, and what to do with
menus that you never click on, but the system is much friendlier now.

In addition I have added a PrintEffects menu to the Notes/Rests menu
which contains the print effect of giving tiny noteheads. The idea will
be to move everything that is simply a print effect to such menus (under
the appropriate objects), so that people don't expect to see it in the
display. Things which just affect the display (e.g. the "wide measures"
command will be moved into a DisplayEffects menu) again so that people
don't expect to see it in the printed page.
