The variables case-replace and case-fold-search have a bearing on this
behaviour.
Using (setq case-replace nil case-fold-search t)
will result in "Realite" => "\index{realite}"
ie. the replacement string is used unchanged.
Using (setq case-fold-search nil)
will result in "Realite" => "Realite"
ie. the string only matches "realite" and not "Realite".
