The parts I can safely answer from memory:

Privately (i.e, before announcements or distribution): November 2001.
(An earlier system was scrapped -- approximately early 2000, if I
recall correctly.)

The first release was self-hosting.   I think that was in January
2002.

That would take some archaeology.   Notable phase shifts might
include:

1) first release
2) first project adoption
3) conversion of `inventory' from a `find(1)' script to a C program
4) the ArX schism
5) tla
6) Perspective and ViewArch
7) Acceptance as a GNU project project.
8) the XOuvert announcement, Emacs mirror, Linux mirrors, etc.
9) somebody asking about the history of the project :-)

Not much hard data on this end.   The quality and quantity of user
submissions of features and fixes has climbed notably.   Adoptions are
certainly climbing but I wouldn't try to put any numbers on it.
