I'd just like to add, that in other fields of endevour (e.g., CEO), when
such massive mistakes are made, it is traditional for the head, who is
ultimately responsible, to take responsiblity and resign.

Remember "The buck stops here"?  Now we have, "it's the CIA's fault"
instead.
