At the top of every source file put

   Foobar, Copyright 2007 Penguin.  See the file "LICENSE" for license
   information.  Nothing else grants any rights.

In "LICENSE" put
   
   Foobar is Copyright 2007 Penguin and is licensed under the terms of the
   GNU General Public License with the exceptions and restrictions listed
   below.  A copy of the GNU General Public license is in the file GPL.  If
   it is missing contact the author or the Free Software Foundation, Inc.
   51 Franklin St, Fifth Floor, Boston, MA 02110-1301 USA for a copy.
