I will assume the "Past Hx of COPD" means to say
that there already existed a *health issue* of "COPD" ?

If so, was there already a (previous) episode under
this issue of COPD ?

And if so, was that previous episode open or closed at the
time you went ahead to "Open a notelet under the issue of COPD" ?
