(I'm on my phone, on holiday and won't be back in front of my computer till Jan 6)
This is my first installation. I played around with the public interface until I felt comfortable and then bootstrapped a local database.
Fist I deleted all the persons from within the GUI and a message warning I could no longer log in popped up. So I had to bootstrap it again.
With me as a new user I repeated the procedure but, as I deleted the last person but me, the bug report dialogue appeared.

I think you did.
Exactly what did you do which way and what happened ?
It seems you are trying to activate from, say, the inbox or
the data mining widget patients which you previously deleted ?
