I have uploaded a very rough sketch of a *working* asynchronous notification 
which I will move into gmPG in due time. Still needs a lot of work, but it 
works ;-)
It is in /gnumed/code_examples/
Maybe it is just good enough to inspire others to start working on it, and to 
insoire the fans of other backend modules (psycopg, pgdb) to work on a common 
API for this.

P.S.: I discovered that "notify" gets hickups when the message contains a 
comma - thus we have to concatenate our message paths with underscores 
instead (patient_selected instead of patient.selected)
