[Gnumed-devel] encounter handling

gmClinicalRecord.__init__() now initiates an encounter. It
checks for "very recent" encounters (age < soft timeout) and
auto-attaches to that. Else it checks for "fairly recent"
encounters (soft timeout < age < hard timeout) and asks the
user whether to continue that encounter. Else it creates a new
encounter of type "chart review".

The user can at any time change the type of the current
encounter via the top panel encounter type selector. Any
subsequent writes will be affected by that change.
