[Gnumed-devel] notebook plugins

Right-clicking on a notebook now allows to load/unload plugins
at runtime again. Two caveats:

- it is always the raised plugin that gets unloaded (as the
  menu item says) no matter which tab was right-clicked
- only plugins that instantiate OK can be loaded

Note that this does not affect plugin load configuration,
changes are lost upon client restart. The plugin "following"
the dropped one is activated upon a drop. A newly loaded
plugin is added at the end.

Easily test this with the "Office" plugin.

Now, plugin developers need not restart the GnuMed client
anymore when hacking plugin code only. A drop/reload will do.
