[Gnumed-devel] history types

While consolidating various tables into clin_narrative some
useful things such es history_type got lost. However,
Elizabeth did quite some research into social history a while
back stating she needed to be able to tag data in various
ways. Also, Richard and Ian talked about social history, past
history, etc. So obviously something like that is needed.

I added to the backend the ability to type any clin_root_item
with an arbitrary number of arbitrary types. This will allow
for tagging as past/social/etc history AND tagging for
subtypes if wanted (education, language, etc etc).

A few predefined types have been added. Views are still missing
as are generic get_types()/add_type() methods on cClinItem
(Carlos, are you interested ?)
