[Gnumed-devel] measurement tooltip

Just for good measure I added a few clinical evaluations.
The tooltip will now tell

- how many times the upper normal/clinical limit the value is
        or
- which percentage of the lower normal/clinical limit the value is

- how many times of the normal/clinical range the value
  deviates from the range

:-)

Say:

normal range = 9.5-10

Result = 11

- result is 1.1 times the upper limit
- result deviates by 2 times the normal range

Result = 8.5

- result is 89 % of the lower limit
- result deviates by 2 times the normal range

Same with the clinical range.
