I envision this to parse my input and magically know. Seriously this will be
tough. I would ty to make that semi automatic.

If one types

patient reported to have had a BP of 200/100 this morning at home I would not
trigger anything

If one types

patient reported to have had a \m{BP,200/100,today}
or \m{BP 200/100 today}

on entering a space before typing 'at home' or whatever follows the first part
I would try to parse that into a measurement, send it to a intermediate
inbox/textbox type field below the soap area

right clicking on the supposed to be / auto detected measurement lets the user
edit and save as measurement

on saving a soap note the user the user will be prompted to have a look at the
preparsed stuff if she has not done so already.

I believe a popup the second something is detecte will be a pain in the a**

Just my 2cents
