GNUmed already stores multiple names and addresses per distinct person.

The names provide for recording whether (which) one is active or "preferred".

I don't see a similar provision for addresses, however they do support a 
comment which could inform "old" or "parent's address (school break)

in v15 now obsoleted by

        dem.identity.fk_primary_provider

??
