(WAIT, _WAIT_CHILD) [!_MSC_VER]: Define.
* src/preproc/html/pre-html.cc (waitForChild): Use WAIT.
* src/preproc/html/pushback.cc: Include nonposix.h.
* src/roff/groff/pipeline.c: Define strcasecmp and strncasecmp
conditionally.
