I think this is because it's not really something that many
users badly require. (I certainly don't.)
Continuous underlining can probably only be done well by
treating it as a character attribute in the formatter and
passing it on to the postprocessor for execution
(like color, for example).
It might be possible to get a reasonably well working
implementation in a macro package by diverting the text and
inserting start/stop markers, and then trapping the formatted
text line-by-line and drawing the underlines. I don't know
whether this extra effort is really worth it, and how well it
integrates into the rest of all the processing that the macro
package performs. (But ultimately, text hyperlinks in PDF
suffer from the same problem.)
