Ah, it's much simpler than I thought.

I assume between releases we'd add a counter after the version number?  Like
grub_abi_ver_1_96_X or so?

Yes. Symbol should be changed with each incompatible ABI change. Not necessary for simple ABI enhancements like new functions.
