Details:
Up to now we've been using an Integer field type for patient Glasgow coma
scale.
We need to make it descriptive, and it needs to be chosen from a selection
list. This way we avoid typing errors, we have the description of the scale
for Eyes, Verbal and Motor section and it will automatically update the final
score.
