Details:

GNU Health HMIS node will use FODT format for reports, (see task #15720: HMIS:
Convert the reports to FODT format). 

Most reports work OK, but I've noticed that those who involve charts with
tables (summary report, epidemiological surveillance )are not rendering the
graphics.

At this point, for those specific reports, we would use the standard ODT
format until a fix is found.

The following changeset includes the odt files for those conflicting reports.
