in your scenario, you use the state "Checked in" as for those that have
already paid.

As per the invoicing, you need to know what is the product / service
associated to that appointment type (different appointment types would
have different fees). You could also have a generic appointment with a
fix fee. I guess that depends on the rules of the institution. 

Once you have those services setup, you can create and assign the
service document and its service lines to the patient.

Automating the services is an old debate, that we should improve
usability without bloating the system, and keeping it as generic as
possible. At this point, we have automated the presciption, we could do
the same for complementary tests (lab, Dx imaging),
hospitalization, ... I think it's institution dependent, but I also
think that there is room for optimization :)
