It is in the dvi, ps, and pdf versions.  I don't see anything
like this in the html or info.

My wild guess is that you have an empty @top section.
In other formats, it has the menu. I suppose if we had written something
to go here, the issue would have been more obvious.

How can I get rid of it?

Should I also include the @menu... @end menu in this?

Do people include a top with text in printed formats? Seems strange to me.
