the problem is that your mpeg1 stream seems to lack access point
sectors; mostly due to missing sequence headers before GOP headers..
please see also the vcdimager manual regarding this issue...

you have 3 options so far;
1) find a way to add those sequence headers in the video stream...
2) use the "relaxed aps" option (may lead to problems on some playing
devices)
3) don't use entry points at all (but then chaptering is out of
sight...)
