In our case, this was done intentionally. All directory names were
renamed to lowercase names. As we did not know about this CVS problem,
this did not seem being a problem beforehand.

On our system, it wasn't an issue unless you checked out by date.  Checkout by tag/revision worked ok.  Proably because the tag was always aplied to the correct file name and not to the errant file.
