The only required fields for a basic setup should be:

1. Hostname (hostname of your SIP server)
2. Username (username used to connect to your account)
3. Password (the password paired to your username).

This should get you started. Other options are service (Cisco) specific which I 
don't know, but you should be able to start experimenting without those.
