improved lilypond-book.py

Attached is a new version of lilypond-book.py, a completely
revised glossary.tely, and an updated lilypond-book.itely.

Note that the revision of glossary.tely affects mainly optical
appearance, not the contents.

The next step will be the fixing of the \parindent problem.
