All I can say is I _can_ reproduce this from _your_ PDF only. I obtain

and 7 more similar to the last one.  Otherwise our PDFs look nearly
identical but I don't have any strange output from my minimal document
/ my hand-compiled lilypond from git. / my PDF / my evince.
