This is normally 1.0, but if you have a book where some scores have a
differently sized staff, they will have a smaller number there.

IIRC, we scale up/down the units so they are expressed in staff space,
so if you do

(ly:output-def-lookup paper 'mm)

you'd get 1mm expressed in staff space
