I still don't understand why you don't use a simple latex or postcript
command.
However, you are remarkably focussed to get this to work, so here are
the gory details about page layout. You have three options.
1. LilyPond <= v2.2.x does not do page layout, it lets [La]TeX handle
the problem. When you run
lilypond --keep foo.ly
the lilypond.py script creates the latex wraper
lilypond.dir/foo.latex. That's about the only place you can do page
layout.
2. Write the latex wrapper yourself and use lilypond-book.
3. Track/wait for/join lilypond development. LilyPond 2.3 has page
layout. Customisation of margins, headers and footers are not yet
implemented.
