Don't you have to specify a tempo? (not sure what happens if you don't)
\midi {\tempo 4 = 60}
specifies the midi file will play a quarter note at 60 beats per minute.
If memory serves...I'm at work - no access to my code files - so I might be wrong with some of the details. My memory is a little unclear if the spaces in "4 = 60" are necessary or not.
