I wonder if anyone discovered a toolchain that would allow to

1. scan sheet-music (e.g. solo transcriptions with many many notes
   ...)

2. convert the scanner-output to lilypond (maybe via an intermediary
   format like xml or so)?

and what kind of software they used for it?

My toolchain is slightly longer, but I don't see why this could not be truncated.

I use SharpEye (http://www.visiv.co.uk/) to generate XML. For historical reasons I then import the MusicXML into Noteworthy composer, then export from that as Lilypond. The amount of correction required depends on how good the original music was.

There's also PDFToMusic

http://www.myriad-online.com/en/products/pdftomusic.htm

which can play the music and export to MusicXML.
