Or perhaps different LilyPond versions? Note that the code requires a recent "unstable" version of LilyPond, i.e. version 2.19.something. It won't work with version 2.18. In addition to the answers you already received, I guess that reading http://lilypond.org/doc/v2.19/Documentation/learning/explicitly-instantiating-voices will help.
