It’s a bug. According to https://sourceforge.net/p/testlilyissues/issues/5179/ the second dot should be killed.
But that’s not the only problem with this engraver and dots: Sometimes, dots are placed randomly, see https://sourceforge.net/p/testlilyissues/issues/5217/
