It must be near to / in the code where lwip is initialised.
E.g. just before setting the manual IP or starting the DHCP client.
There should be a list of some calls into lwip
where functions named *_init() are called.

I cannot tell the filename, since it can be
anywhere in your application.

Sorry, no idea. I'm not an expert on the NIOS,
maybe you can ask Altera support.
