Actually, it would depend on the speech output software being used. Some
packages can be "trained" or configured to automatically speak when a box
appears on the screen. Also many speech packages can track a highlight
bar so if the selection box has a light bar in it, it will automatically
be spoken when the cursor keys are moved for up and down movement. I
don't own a braille display so I will defer answers about them.
Personally, I use Vocal-Eyes for DOS and Window-Eyes for windows
applications.
