Yes.

Yes. Lieu-dit or Hameau is (normaly) never in the name value. May be you can 
avoid adding 
Lieu-dit if the index title clearly shows 'Index des rues et lieux-dits'. 
If you add it avoid the case "Lieu-dit Lieu-dit Tartenpion".
