Follow-up Comment #3:
This appears to be a gnuplot bug, rather than an Octave bug.
In the intervening years, the situation has gotten better. With the
development branch of Octave and gnuplot 4.6.2 I no longer see problems 1 and
2. There are still some lines extending beyond the axis boundaries, but,
again, this seems to be the fault of gnuplot. If I change the x-axis limits
from [-1 1] to [-1.0001 1] then the figure draws correctly.
