Details:

"help hess" gives:

  H is upper Hessenberg ('H(i, j) = 0 forall i >= j+1)'

This should be:

  H is upper Hessenberg ('H(i, j) = 0 forall i > j+1)'

For example:
hess(rand(4,4))
ans =

   0.83253  -1.31666  -0.23068  -0.20762
  -1.11475   1.35071   0.51557   0.09863
   0.00000   0.90453   0.31000  -0.32957
   0.00000   0.00000   0.13293  -0.52310
